Dear Mr. Smith,

I am writing this letter to submit my request to you, because I need to recover from the car accident last Wensday. After the doctcor told me my rehabilitation plan, I would like to tell you my conditions and subsequent work.
It was a serious car crash that day, a truck on the left hit my car, my right leg was broken since it was stuck with the brake when I wanted to stop. Now I am still at hospital where the doctor told me it would take three months to recover. This is a long time but I do not want to delay my work as well as the project I am working on.
