Dear Sir or Madam,

I am writing to you to file a travel insurance claim. my policy number is CP2019TR469 covering my DSLR camera that I lost recently while staying in berlin. Its a new nikon D35 camera which costed me US$400 from the only nikon showroom located in berlin.

Yesterday, while I was having my dinner in a local restaurant, a young couple sat in-front of me. Though they left my table before I have finished with my food, I could not find my camera that I kept beside me. I already complained to the nearby police station, the paper of the case is attached along with the purchase voucher of the camera.

I hereby request you to reimburse US$360 as per my insurane policy to cover 90% of actual cost of lost items. As I currently do not have my account number, please consider the bank account that I already mentioned in the policy paper for the payment.

I look forward to receiving the claimed money in my bank account at your earliest.
