Dear Sir or Madam,

I am writing to express my dissatisfaction with the recent meal we had at your restaurant.

Every week on Thursday, we have team members dinner at your restaurant from the past 9 months. We were very happy with your food and services.

However, during our last meal on March 21st, the service we received was not up to the mark. There were many problems. To being with, the soup we ordered was cold. Another issue was the salad was not fresh. It seems that the ingredients were stale and overly salted to curb the taste of staleness. Lastly, the service was very slow, even though we had reserved the table well in advance. When I asked the waiter the reason for the delay, his attitude was not only quite impolite but also he did not give a proper answer.

I would recommend that you check with your kitchen staff since they are not doing a job properly to ensure that the food that is being served is up to the mark and quality is meeting the price you are charging. Lastly, you must train your serving staff to be more polite.

My team will closely check all the services you will be proving in our next group dinner. If there is any shortcomings, we will be forced to change our venue for dinner, and you will lose a loyal customer.

I hope you will take corrective actions to ensure this problem is not repeated.
