Write a letter to the manager of your bank. In the letter
explain how you lost the card
ask the manger to cancel the old card and to issue you a replacement

Dear Sir,
I am writing to inform about loss of my credit card (Card Type: master, Number: 2123987655) which was issued by your bank in September 2017.
Generally I use the credit card on special occasions and 21st April2018 was my 15th marriage anniversary so I decided to use it for grand dinner. I and my wife went to dinner at city center famous restaurant “Haveli” where I paid the amount through credit card. I remember putting the card back in my pocket, however, when I reached home, I did not find the card. It seems like that I lost it during commute to my house from city center.
Though I reported the card loss to lost & found department at police station but I have less hope of finding it. Moreover, I suspect that card could be used by other people on internet transaction where pin is not required. I request you to immediately block my card and issue a new card with chip feature.
I am looking forward to hear from you
Yours sincerely
Purwar
